Save the completed form by printing to .pdf; then upload the form to your Synergy application. <\/p>\n\n\n\n<p>For Non-Standard Testing Form 1 and Forms 3 and 4, if applicable, upload the completed form to the Request for Non-Standard Testing, <strong>not <\/strong>your exam application. Forms uploaded to your exam application will not be reveiwed by the Board as part of your request for Non-Standard Testing Accommodations.
